  • If you are wanting to use your insurance, a referral from your PCP, OB/GYN, or chiropractor is required. Your referring provider can also send any supporting documents that you feel are necessary for us to know to better serve you.

    Your provider can fax a referral to 541-306-3051.

  • Yes and No

    DPHW is credentialed with Medicare Part B, Open Card OHP, and all plans under PacificSource including Commercial, OHP, and Medicare Advantage.

    If you have another form of a Medicare Advantage Plan, DPHW is able to bill that insurance as long as you have out of network benefits.

    All other health plans will be considered out of network. Please call your health insurance company to clarify if you have out of network benefits. If you do, DPHW is able to provide you with a superbill to submit to your health insurance in the attempts to be reimbursed.

  • Please call your insurance company to know if you have out of network benefits. If you do, DPHW can give you a super bill so you can submit the claims to your insurance provider in hopes to be reimbursed.

    DPHW cannot guarantee that your insurance provider will reimburse you.

  • Free parking: There is free street parking one block west of the clinic in the residential section of W Broadway.

    Paid parking: There is paid metered street parking right in front of the clinic. There is paid garage parking on the top floor in both garages at the corner of Charnelton and Broadway.

  • Welcome! We are so glad you are here. During your first appointment, you will have 90 minutes to discuss your concerns and get insight into how Dr. Chrissy can help you.

    It is advised that you wear whatever is comfy to you so you can move around freely.

    A typical session includes:

    • Thorough discussion of your past medical history and current concerns

    • Education on anatomy and pelvic floor function

    • Physical exam (an internal pelvic exam may be appropriate depending on your concerns and comfort level)

    • Discussion on findings of physical exam

    • Education on homework to help improve function

  • Dr. Chrissy uses the local listening and the LTAP (Locator Test Assessment Protocol) created by Anna Hartman to help direct our session. These techniques will help Dr. Chrissy determine where in your body needs attention first to insure a more successful session.

    The treatment techniques used during each session may differ depending on your needs and progress, however, you should expect these four components during each session:

    • LTAP

    • Manual therapy

    • Education

    • Exercise

  • YES!

    You can have your kids in the treatment room during your appointment. If they are older kids, they are able to sit in the lounge on the couch.

    If you are a mom who is breastfeeding and would like to have a child free appointment, you are more than welcome to have a caregiver hang out in the lounge area with your baby in case they need you.
